This policy applies to employees, visitors and students.

1. Notify your supervisor immediately!
2. Report to University Health Services(UHS) for medical treatment.
3. Inform the medical provider at the time of treatment that this is work related injury.
4. Complete a UC accident report [A-1352 (a)]. You can download the form at Environmental Health & Safety with your supervisor within 24 hours.
5. Call UC Benefits at 556-OUCH (556-6824) for workers' compensation information.

1. In the event of a medical emergency, call 911.
2. Seek treatement at University Hospital, Incorporated, or at the nearest available medical facility.
3. Follow steps 3, 4 and 5 above.
